For Teens and Young Adults with Type 1 Diabetes

In January 2010, Gales Creek Camp hired a Transitions Specialist to coordinate and expand the services for teenagers and young adults with type 1 diabetes – both campers, and other emerging adults with diabetes. This program will be a unique service, and we envision GCC as a leader in bringing together community members and health care organizations interested in better serving this special group of emerging adults. |

Research shows this age group has unique needs and challenges in their diabetes self-management.
We are here to help!
Currently, we are conducting a needs assessment with pediatric and adult health care providers, as well as with twenty-something adults who have successfully transitioned with diabetes, and parents of teens with type 1.

The Transitions Specialist will be available at High School Camp this summer (June 29 – July 3) to meet with teens and parents regarding healthy, successful “life after high school.”
